Support Coordination
A support coordinator is a professional who helps you to get the most out of your plan. If you have plan managed funding, or self-managed funding in your NDIS plan, All Unique Community Care can help you with support coordination.
A support coordinator will help you:
- Understand your NDIS plan
- Help you access the supports in your plan
- Find the right service providers for you
- Work with your service providers to support you in working towards your goals
- Work with you to establish your goals and what you want out of your NDIS plan

The Dandelion
The dandelion was chosen for our logo due to it symbolising hope, love, happiness, resilience, and freedom. The dandelion is able to survive anything from harsh winters, pollution, drought and being trodden on. They can quickly bounce back from adversity and continue to grow.
